Requesting Accommodations During Clinical Rotations Authorization Form

• If you are requesting for your accommodations to be carried over to your clinical rotations, you must provide authorization so that MBKU can share your accommodation request and disability-related needs with your preceptors. Please fill out this form and send it to studentaffairs@ketchum.edu.
